Registering with the SNS: Portugal's Public Healthcare for Newcomers

Get your SNS number, enrol at a health centre and check your coverage — three different tasks. A number alone does not guarantee a family doctor or SNS-funded care.

Procedure checked against official sources on 2026-10-07. Check live availability before you act.

Key takeaways
  • The SNS number is unique and definitive. Visitors and people without regularised status can also have one; it does not by itself establish who pays for care.
  • Register in person at your chosen health centre, preferably near home. An updated RNU record is separate from primary-care enrolment and family-doctor assignment.
  • Adult foreign-resident documentation differs from the rules for foreign children under 18: NIF and residence-permit fields are not mandatory for those children.
  • Temporary visitors should check valid EHIC or other insurance entitlements. People moving country must check the responsible healthcare system and any applicable S1 route.

Start by checking whether you already have an SNS user number, then take your situation-specific documents to a health centre. Ask the desk to confirm both your enrolment and the payer recorded for your care. These are not automatically settled by receiving a number, and they are separate from being assigned a family doctor.

SNS number, registration and coverage: what is the difference?

ItemWhat it means
Número nacional de utente (NNU)Your unique, definitive SNS identifier. Missing documents do not make it provisional.
Registo Nacional de Utentes (RNU)Your identity, address, status and payer record. Check whether it is atualizado, atualizado não residente, em curso or incompleto.
Primary-care enrolment (CSP)A separate registration at a health unit. Ordinary enrolment requires an updated or updated-non-resident record.
Family doctorA separate assignment subject to eligibility and vacancies, not guaranteed by an SNS number.
Financial coverageWhether SNS or another entity pays, or you bear costs. This is not the same question as a user-fee exemption.

Which documents do adults, children and visitors need?

  • Portuguese Citizen Card holder: take the card, address details and evidence of any benefits. The number normally already exists; ask the centre to locate it.
  • Adult foreign resident: bring civil identification, NIF, full Portuguese address and the residence evidence for your situation. If address evidence is needed, ACSS lists a lease, loan-for-use agreement or water/electricity/gas bill as alternatives. Take evidence of any payer entitlement or exemption you claim.
  • EU resident: take the applicable municipal registration evidence. ACSS accepts an expired CRUE for the RNU record with current address verification; that health-record instruction is not a blanket extension of every immigration document.
  • Foreign child under 18: NIF and residence-permit fields are not mandatory for an updated record; civil identification is required after two months of age. Ask about the child's specific access evidence rather than applying an adult checklist. Special protection routes have their own evidence requirements.
  • Temporary visitor or person without regularised status: a number can still be assigned. Tell the unit about the insurance, agreement or special access route that applies; do not assume an identifier means all treatment is publicly funded.

For the ordinary tax identifier, use the NIF request guide. EU newcomers can check the municipal CRUE procedure. For health-centre documents and the current channel, open the official registration service.

How to register and verify the result

  1. Find your existing SNS number first

A Portuguese Citizen Card normally generates the number automatically. Otherwise ask a public health unit to find or create your record. The number is unique and definitive, including when information is missing: do not ask for a second or provisional number.

  2. Prepare the documents for your situation

An adult foreign resident should bring identification, NIF, full Portuguese address and the residence evidence applicable to their status, plus evidence of any benefits or payer entitlement. Foreign children under 18 do not need NIF or residence-permit fields for an updated RNU record; civil identification is required after two months of age.

  3. Attend the chosen health centre

The current gov.pt service lists an in-person channel, preferably at a centre serving your address. Call SNS 24 on 808 24 24 24 if you need help finding it. If you have no number, the centre can assign one; identifier assignment and centre enrolment are separate decisions.

  4. Confirm your RNU record, enrolment and payer

Ask which RNU type is recorded, which information is missing, whether you are enrolled in primary care and who is responsible for payment. An incomplete record is not a 180-day free-care entitlement. Keep the request or confirmation and complete any missing evidence.

  5. Check the family-doctor and consultation route

Ask whether a family doctor has been assigned or you are waiting for a vacancy, and how to obtain care meanwhile. Update your contact details and address when they change. Correcting an RNU record does not automatically restore a primary-care enrolment that was ended: ask the centre to check it separately.

Can you request NIF, NISS and SNS together?

The free combined request is made in person at selected Espaços Cidadão, not through a universal online form. It is for foreign citizens with an address in Portugal, without a Portuguese Cartão de Cidadão, who are EU citizens, have applied for or hold a residence permit, have applied for international protection, or have refugee/protection status. Check the official counter list and opening or appointment arrangements before travelling.

  • Bring a passport or an EU-country identity document and proof of the eligible status: for example, the AIMA application receipt, residence title or municipal EU registration certificate. An appointment alone is not listed as the residence-application receipt.
  • If that status document has no address or a different address, bring matching evidence such as a lease, loan-for-use agreement, utility bill, property-register certificate/code or landlord occupancy statement.
  • A person applying for themselves authenticates with passport-linked CMD or eIDAS. If needed, CMD can be created with the passport at the counter. A representative has separate CMD/CC/eIDAS and power-of-attorney requirements; a parent or guardian applying for a minor must bring the minor and proof of the relationship.
  • The service sends emails about submission to AIMA and the decision. Approval can cover all numbers, only some, or none: do not assume three numbers are issued immediately. Check assigned numbers in the gov.pt reserved area or app and keep the decision.

Check eligibility, documents and counters on gov.pt. Receiving an SNS number is not the same task as registering with a health centre or being assigned a family doctor. Ordinary NIF and NISS requests remain separate alternatives.

If only some identifiers are approved, check which are missing before reapplying. Use the standalone NISS procedure for that request and the CMD identity guide for authentication. These do not replace health-centre enrolment.

What if the RNU record says em curso or incompleto?

Em curso means required information is missing. The legal 180-day period runs from the SNS contact requesting updated data; it is not a visitor's free-care period or an immigration deadline. The record becomes updated if the requirements are met, or incomplete if the missing information is not supplied. These types ordinarily mean a third party or the patient bears costs, subject to the applicable exceptions, and do not support ordinary primary-care enrolment.

Ask exactly which field is missing and how to prove it. After correction, check the financial record and enrolment separately: ACSS says an enrolment that was ended is not automatically restored. An expiring residence document generates record alerts and requires verification; do not treat that as an automatic loss of the SNS number or proof that immigration permission has been extended.

Atualizado não residente is specifically for Portuguese citizens normally living abroad who satisfy the other updated-record requirements. It allows primary-care enrolment under the current rules, but not family-doctor assignment. It is not a general category for every foreign tourist. Histórico is for a deceased user, not simply someone who moved abroad.

Who pays, and are there still user fees?

The number alone does not establish SNS financial responsibility. For an updated foreign-resident record, the applicable SNS funding rules differ from those for a visitor or an incomplete record. ERS lists exceptions including maternal/reproductive care, qualifying resident minors, national vaccination programmes and certain verified family-reunification or social-exclusion situations. Each has conditions; they are not a promise that every visitor receives every service free.

For urgent and vital care without a residence permit, the applicable exceptional route can require a Junta de Freguesia statement confirming more than 90 days in Portugal, presented at each visit. Ask the unit about the precise route and evidence. Do not delay urgent care simply because a registration document is missing.

Taxas moderadoras are different from a full-cost bill. They remain principally for hospital emergency care without prior SNS referral when the patient is not admitted; SNS 24 or health-centre referral, admission and statutory exemptions can remove that user fee. A fee waiver does not itself settle a visitor's or third party's responsibility for the underlying healthcare costs.

EHIC for a temporary stay; S1 where applicable when moving

A valid European Health Insurance Card (EHIC/CESD) from the responsible insurer covers medically necessary public-system care during a temporary stay, on the local terms. Nationality alone does not create that insurance entitlement. It does not cover private care, travel specifically for planned treatment or repatriation, and is not travel insurance.

If you are moving rather than visiting, check which country's system is responsible for your cover. An S1 may apply, for example to certain pensioners, posted workers or cross-border workers; it is not a form every foreign resident can use. Ask your responsible insurer and the Portuguese health unit how the entitlement must be registered.

No family doctor, moving address or a refused registration?

Travelling temporarily from Portugal? Follow the CESD request, renewal and provisional-certificate guide. For proof of a short work absence rather than health-centre enrolment, use the ADD and medical CIT guide; these are separate tasks.

Doctor assignment depends on vacancies; families with pregnancy, chronic illness/multimorbidity or children up to 12 have priority under the rules. Not having an assigned doctor does not remove your right to clinically appropriate care. Ask the unit about its consultation route, then see the SNS appointment guide.

When moving, keep your number, update the address and request transfer of primary-care unit if needed. Phone and email can be updated through SNS 24; identity, address and residence evidence should be checked with the unit. If access is refused, first ask the provider for the reason and clarification. If unresolved, ERS offers information and complaint routes; do not post identity or medical documents publicly.

Frequently asked questions

How do I get an SNS number in Portugal?

Citizen Card holders normally already have one. Otherwise a public health unit can find or create your RNU record, including for a visitor or a person without regularised status. The number is unique and definitive; it does not by itself guarantee centre enrolment, a family doctor or SNS-funded care.

Do foreign adults and children need NIF and a residence permit?

The ordinary updated record for an adult foreign resident requires NIF, address and applicable residence evidence. For foreign children under 18, NIF and residence-permit fields are not mandatory; civil identification is required after two months of age. Special access routes must be checked separately rather than excluding someone solely for lacking a permit.

Can I register with a health centre online?

The current gov.pt service lists an in-person channel at the chosen centre, preferably near your address. SNS 24 online access is not the same as initial centre enrolment. A combined identifier request also does not complete that separate enrolment.

Does an SNS number guarantee a family doctor?

No. Doctor assignment requires the applicable updated record and an available vacancy. The updated-non-resident category for Portuguese citizens abroad does not qualify for a family doctor. Without an assigned doctor, you still have the right to appropriate care; ask the unit how to request a consultation.

Is all care free once I have an SNS number?

No. Check the financial-responsibility record, insurance or other entitlement. SNS user-fee exemptions are distinct from responsibility for the full cost of care. Hospital emergency user fees can still apply without prior SNS referral if you are not admitted, subject to exemptions and waivers.

What do em curso and incompleto mean in RNU?

Required information is missing. Em curso can last up to 180 days from the SNS contact requesting an update; that is not 180 days of free treatment. Ordinary primary-care enrolment is not available with these types and costs ordinarily fall to another payer or the patient, subject to exceptions. Supply the missing data, then separately confirm any enrolment that needs restoring.

Does an expired residence document automatically cancel my SNS number?

No: the number is definitive. ACSS describes alerts and unit verification of residence evidence, not automatic cancellation of the identifier. Ask the unit to verify the current documents, record type and payer. Do not infer from RNU handling that your immigration permission is generally extended.

Can I use an EHIC instead of registering when visiting Portugal?

A valid card from the responsible insurer covers necessary public-system care during a temporary stay on local terms. It does not cover private care, planned-treatment trips or repatriation. When moving country, check the responsible healthcare system and any applicable S1 registration; passport nationality alone is not insurance cover.

Do I need a new SNS number when moving house?

No. Keep the existing number, update your address and request transfer of primary-care unit where needed. Phone and email can be changed in SNS 24; ask the unit to check identity, address and residence evidence. Correcting the RNU record does not automatically restore an enrolment that was ended.

Can I request NIF, NISS and SNS together?

Yes, if you meet the combined service's conditions: foreign citizen, Portuguese address, no Portuguese Cartão de Cidadão and an eligible EU/residence/protection situation. Apply in person at a listed Espaço Cidadão with the required identity, status, address and authentication evidence. The decision can approve all numbers or only some; it is not a universal online or same-day three-number service. SNS registration is a separate task.
